Understanding the Hospital Readmissions Reduction Program (HRRP)

What is an "Unplanned Readmission"?

A readmission occurs when a patient is discharged from a hospital and then unexpectedly admitted back to the same or another hospital within 30 days. While some readmissions are inevitable, a high rate of unplanned readmissions is a major red flag for healthcare quality. It often indicates that the patient was discharged too early, did not receive adequate discharge instructions, or developed a preventable complication like an infection.

The Hospital Readmissions Reduction Program (HRRP)

To combat this issue, the Centers for Medicare & Medicaid Services (CMS) established the HRRP. This value-based purchasing program links payment to the quality of hospital care. By financially penalizing hospitals with higher-than-expected readmission rates, CMS encourages facilities to improve communication, enhance care coordination, and better engage patients and caregivers in discharge plans.

Which Conditions Are Tracked?

CMS doesn't track every single readmission. The HRRP specifically evaluates 30-day readmission rates for six critical conditions and procedures:

• Acute Myocardial Infarction (AMI): Heart attacks.
• Heart Failure (HF): Chronic condition where the heart doesn't pump blood as well as it should.
• Pneumonia: Infection that inflames the air sacs in the lungs.
• Chronic Obstructive Pulmonary Disease (COPD): A group of lung diseases that block airflow.
• Elective Primary Total Hip/Knee Arthroplasty: Joint replacement surgeries.
• Coronary Artery Bypass Graft (CABG): Heart bypass surgery.
