Quality Over Quantity
Historically, hospitals in the United States were paid under a "fee-for-service" model. This meant that the more tests, procedures, and days a patient spent in the hospital, the more money the hospital made—regardless of whether the patient actually got better. The Hospital Value-Based Purchasing (VBP) Program flips this model upside down. It is a CMS initiative that adjusts what Medicare pays hospitals based on the quality of care they deliver.
How Does the Funding Work? (The 2% Withhold)
To fund the VBP program, CMS automatically withholds 2% of the base operating payments from all participating hospitals. Based on a hospital's Total Performance Score (TPS), they can:
- Earn back more than 2%: The hospital performed exceptionally well and receives a financial bonus.
- Earn back exactly 2%: The hospital performed at the national average and breaks even.
- Earn back less than 2%: The hospital performed poorly and permanently loses a portion of its Medicare revenue.
The 4 Domains of the VBP Score
The Total Performance Score is divided equally among four domains (each worth 25%):
healing 1. Clinical Outcomes (25%)
Evaluates patient survival rates 30 days after critical events like heart attacks or pneumonia, as well as the rate of severe complications following hip or knee surgeries.
sentiment_very_satisfied 2. Person and Community Engagement (25%)
Based entirely on the HCAHPS patient experience surveys. It measures how well doctors and nurses communicated and whether patients felt their needs were met.
health_and_safety 3. Safety (25%)
Measures the rates of Healthcare-Associated Infections (HAIs), including MRSA, C. diff, and infections from central lines or urinary catheters.

Evaluates the Medicare Spending Per Beneficiary (MSPB). It measures whether the hospital is providing care cost-effectively without unnecessary tests or extended stays.

No. While Efficiency and Cost Reduction is worth 25%, the remaining 75% of the score is entirely dependent on keeping you alive, safe from infections, and satisfied with your care. If a hospital cuts corners, their Clinical Outcomes and Safety scores will plummet, causing them to lose far more money than they saved.

Do all hospitals participate in VBP?

No. Only Inpatient Prospective Payment System (IPPS) acute-care hospitals participate. Critical access hospitals (small rural hospitals), psychiatric facilities, children's hospitals, and rehabilitation centers are generally exempt.
